Disability Insurance Carriers
Five major carriers dominate the individual disability insurance market for professionals. Each has different strengths, contract language, occupation class assignments, and pricing. As independent brokers, we compare all five for every client — and we never steer you toward any one company.

The individual disability insurance market for high-income professionals is concentrated among five carriers, often called "the Big Five." All five offer true own-occupation coverage for qualifying occupations, non-cancelable and guaranteed renewable policies, and a full suite of riders. The differences lie in contract language details, occupation class assignments, available discounts, underwriting guidelines, and pricing for specific situations.
Guardian Life
Guardian is widely regarded as the carrier with the strongest own-occupation contract language available — their "Enhanced Own-Occupation" definition is particularly valued by physicians and procedural specialists. Guardian also offers Guaranteed Standard Issue (GSI) programs at many residency programs and medical schools, allowing trainees to obtain coverage without medical underwriting.
Best for: Physicians, surgeons, and procedural specialists who prioritize the strongest possible definition of disability. Also strong for dentists and attorneys. Financial strength: A++ (AM Best).
MassMutual
MassMutual is one of the oldest and financially strongest insurance companies in the United States, with a COMDEX score of 100 — the highest possible composite financial strength rating. Their disability policies feature strong own-occupation language and a unique RetireGuard rider that provides retirement savings protection during a disability. MassMutual also participates in many GSI programs for medical professionals.
Best for: Clients who prioritize carrier financial stability above all else, and those interested in the RetireGuard retirement protection rider. Financial strength: A++ (AM Best).
Principal Financial Group
Principal offers flexible policy design with strong residual disability provisions, making them particularly competitive for high earners and self-employed professionals with variable income. Their policy structure accommodates business owners well, and they often offer competitive pricing for certain occupation classes that other carriers rate less favorably.
Best for: Business owners, high earners, and self-employed professionals. Often competitive for occupations where other carriers are more restrictive. Financial strength: A+ (AM Best).
Ameritas
Ameritas is frequently the most price-competitive option among the Big Five for certain occupation classes, particularly for physicians and dentists. Their contract language is solid, and they are often used as a cost-effective complement to a Guardian or MassMutual policy when stacking coverage across multiple carriers to reach higher benefit amounts.
Best for: Cost-conscious buyers who want strong coverage at competitive pricing, and as a supplemental carrier when stacking policies. Financial strength: A (AM Best).
The Standard (StanCorp)
The Standard provides solid individual disability coverage and is particularly strong in certain states and occupation classes where the other four carriers are less competitive. Their policies include a strong recovery benefit and can be a good fit for clients in occupations or health situations that receive less favorable treatment from other carriers.
Best for: Clients in certain geographic markets or occupations where The Standard's underwriting and pricing are most competitive. Financial strength: A+ (AM Best).
Why Independent Comparison Matters
A common misconception is that buying directly from the carrier saves money. It doesn't — individual disability insurance premiums are filed with state regulators and are the same whether you buy through a broker or directly. The difference is that an independent broker compares all five carriers simultaneously, identifies the best contract language for your occupation, and finds available discounts (multi-life, association, unisex) that can reduce premiums by 15–30%.
Carrier-employed agents can only show you their company's products. We show you all five.

For physicians evaluating which carrier fits their specialty and stage, head-to-head comparisons cut through the marketing noise:- Guardian vs. MassMutual — the two strongest mutual companies in physician DI; differences come down to surgical 6M class vs. mental/nervous parity coverage.
- Guardian vs. Principal — the most common Guardian alternative for residents and non-surgical specialists; Principal typically wins on pricing and student loan rider.
Carrier Coverage by Specialty
Different carriers prioritize different specialties. The right carrier for a surgeon is rarely the right carrier for a resident or a CRNA:- All physician specialties — top-class 5M and 6M coverage
- Medical residents and fellows — pre-attending coverage with future increase options
- All dental specialties — top-class 4D and 5D coverage
- CRNAs (nurse anesthetists) — 4M class with mental/nervous parity
- All healthcare professionals — NPs, PAs, pharmacists, podiatrists, optometrists, and more
Key Policy Concepts to Understand Before Comparing Carriers
Before evaluating carriers, two concepts decide which one fits your situation:- True own-occupation — what it means and why it matters more than carrier name for surgeons and procedural specialties
- Disability insurance policy basics — the riders, definitions, and benefit structures that vary across carriers
- Group LTD vs. individual coverage — why hospital-provided LTD is rarely sufficient on its own
